Position Summary:

The CLTS program supports children with disabilities and their families by offering tailored services that enhance their quality of life. As an integral part of the Youth Treatment Team, the CLTS Case Manager is responsible for planning, coordinating, and delivering case management services. This role involves developing individualized service plans, monitoring services, and maintaining a collaborative relationship with clients, families, and community partners.

Responsibilities:

  • Conduct assessments, including the Children’s Functional Screen, and ensure compliance with program guidelines.
  • Develop and implement Individual Service Plans (ISPs) that prioritize the child’s health, safety, and developmental needs.
  • Coordinate service proposals, authorize services, and monitor provider agreements within CLTS Waiver guidelines.
  • Serve as a liaison between families, service providers, and state offices to address client needs.
  • Allocate resources effectively based on assessment outcomes and ISP requirements.
  • Maintain regular contact with families and provide ongoing support to address issues and concerns.
  • Ensure case files and documentation meet established standards and timelines.
  • Actively participate in team meetings and community partnerships to enhance service delivery.
  • Provide updates and represent the program at Children’s Advisory Committee meetings.
  • Perform other duties as assigned, including emergency response when necessary.
  • Demonstrated ability to collaborate with diverse stakeholders, including families, service providers, and community organizations.
  • Knowledge of trauma-informed care principles and strategies.
  • Proficiency in documentation and compliance with program standards.
  • Strong problem-solving and crisis intervention skills.
  • Commitment to promoting respect, optimism, integrity, compassion, and collaboration in all interactions.

Qualifications and Competencies:

  • Bachelor’s degree in a health or human services-related field.
  • Minimum of two years of professional experience working with children with disabilities and their families, preferably in community programs.
  • Knowledge of Medical Assistance (MA) Waiver Manual, community programs, and relevant state and federal regulations.
  • Excellent communication and organizational skills, with the ability to manage multiple tasks effectively.
  • Experience in waiver programs and functional screening preferred.
  • Valid Wisconsin driver’s license required.
  • Excellent assessment and interviewing skills as well exercise good judgment
  • Ability to work well with individuals from diverse economic, racial, and ethnic backgrounds and LGBTQ+ identities
  • Pre-employment background check and annual tuberculosis testing and flu vaccinations as required by Milwaukee County
