What You’ll Do

As a Service Coordinator, you’ll be at the heart of a child’s journey, helping families access the support they need and achieve the goals outlined in their Individual Family Service Plan (IFSP). Here’s a snapshot of your responsibilities:

Empower Families and Collaborate with Teams

  • Work with a team of professionals—including parents, therapists, social workers, and teachers—using the Primary Coach Approach to Teaming (PCATT) model.
  • Provide coaching and intensive family support to help families address concerns, frustrations, and needs.
  • Coordinate assessments to determine eligibility for programs and services.
  • Facilitate the development, review, and evaluation of the IFSP with the family and team.
  • Conduct home visits and maintain monthly contact with families to ensure open communication and support.
  • Identify service providers and connect families with the resources they need.

Coordinate Services and Advocate for Families

  • Facilitate access to services, monitor their provision, and ensure timely communication with team members.
  • Provide inclusive documentation and accurate billing for all work.
  • Keep systems like PPS updated to reflect each child’s services.
  • Assist families with transition planning, including preparing for Individual Education Plan (IEP) meetings.

Train and Support Staff (If Assigned)

  • Train new staff in PCATT principles and processes, including evaluation, IFSP development, and documentation.
  • Model best practices in coaching, documentation, and billing.
  • Provide feedback and develop a 90-day review process for staff.

Administrative Duties (If Assigned)

  • Process referrals and ensure compliance with state systems.
  • Assist with auditing and documentation requirements.

Qualifications

  • Strong communication and interpersonal skills.
  • Experience working with families and children, especially in a service coordination or social work capacity.
  • Familiarity with Medicaid, licensing standards, and Birth to Three State and County standards is a plus.
  • A passion for empowering families and building supportive relationships.
