Case Manager -Youth Homelessness Demonstration Project (YHDP)

Job Purpose and Reporting Structure:

In partnership with Milwaukee Coalition on Housing and Homelessness (MCHH), with Coordinated Entry and Walkers Point, Youth Action Board, and under the direction of the Program Manager, the Case Manager provides assistance to young adults enrolled at ages 18-24 who are seeking resources and support through Milwaukee’s HUD funded YHDP. This support offers individual case management for the joint rapid rehousing/transitional housing funded project,, and access to supportive services provided by internal and external partners.

Responsibilities:

  1. Assess housing barriers of individuals and families experiencing homelessness to determine their housing and service needs.
  2. Work cooperatively and effectively with Youth Action Board, Walkers Point, MCHH, Coordinated Entry, Pathfinders Supported Housing services, Drop-In Center, Youth Anti Violence Services, emergency youth shelter, and the Street Beat street outreach s in order to coordinate services and maximize youth access to necessary resources.
  3. Provide intensive and ongoing case management services to 20+ participants at a time, focusing on finding and helping participants to maintain safe and secure housing.

    a. Assist participants in the development of strength-based action plans that are individualized and youth-centered and that support the youths’ development of Positive connections, Academic achievement, Training and employment, safe and secure Housing, and Social and emotional well-being.

    b. Ensure low-barrier access to housing while also providing youth choice with where they are housed.

    c. Conduct home visits in the participants’ apartments so that participants can work toward housing stability and a provision of life skills while working toward the goals they set for themselves.

    d. Serve as an ongoing liaison between participants, Pathfinders housing specialists, and other team members.

    e. Create and implement a move in plan in collaboration with participants, and transition plan including aftercare and graduation services.

  4. Increase participants’ access to mainstream community resources available within the Greater Milwaukee area as outlined in the comprehensive community plan, working effectively with designated partners to ensure successful service access.
  5. Attend required HUD YHDP specific technical assistance, trainings, meetings, etc,…
  6. Adhere to HUD regulations, YHDP Round 7 compliance, Comprehensive Coordinated Plan (CCP), Continuous Quality Improvement plan (CQI), and individual service plans.
  7. Maintain client records, keep accurate case notes, and complete necessary client reports in internal and external required information systems.
  8. Participate in all relevant and required external and internal program, team and agency meetings, trainings, and development opportunities.
  9. Other duties may be assigned.

Qualification Requirements:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience:

Education generally equivalent to a Bachelor’s degree in a human services field is preferred. Lived experience is also valued and will be considered when selecting candidates. All candidates must use a low barrier and immediate access to housing approach.

Language Skills:

All candidates must be able to read, analyze, and interpret professional journals, technical procedures, and governmental regulations. They must also be able to write reports and business correspondence while working within budgetary expectations. Candidates must possess the ability to effectively present information and respond to questions from groups of managers, clients, customers, professionals, and the general public.

Mathematical Skills:

All candidates must have the ability to perform basic high school mathematical operations, which includes general finance activities. Basic knowledge of office software, including word-processing, database systems, and spreadsheet programs is also required.

Reasoning Ability:

Candidates must have the ability to apply common sense in order to carry out supervisory direction and instruction that comes in both written and oral form. The ability to problem solve in situations involving several abstract variables is also necessary.

Certificates, Licenses, Registrations:

Candidates must have a valid Wisconsin driver’s license and proof of automobile insurance coverage as well as a reliable vehicle. First aid certification is recommended but can be obtained after hire.

Technology Skills:

Proficiency in Microsoft Word, Excel, and Outlook computer programs is required. The ability to use office equipment such as a computer, fax machine, telephone, and copier are also needed.

Other Skills and Abilities:

Candidates must have experience working with a diverse population of adolescents and young adults. They must possess excellent communication, interpersonal, and conflict resolution skills and must be able to work, both independently, and as part of a team. They must also have the ability to maintain client records/files and generate reports for use by others as requested by a supervisor. Due to the nature of the work, candidates must be able to work in a changing environment where there is frequent exposure to stressful/crisis situations. There must be a willingness to work a

flexible schedule of morning, afternoon, and evening hours as needed. Candidates must be organized and reliable and must be able to maintain privileged client information, while working effectively within nontraditional settings. A desire to work with a diverse population of adolescents and young adults, including LGBTQ+ youth, is needed.

Additional Information

Position pays $40,000 – $50,000, Pathfinders offers medical, dental, vision and insurance, and the opportunity to join a caring and committed team of professions.
