POSITION DESCRIPTION

This position is a full-time, salaried position that will work with the Milwaukee Family Intervention Services (MFIS) Project. This position will be the primary point of contact for families. Services will occur primarily in the home however contact can be arranged with the family to suit their individual needs. Emphasis will be placed on building on the family’s strengths while seeking to control or stabilize those conditions, which threaten the family stability. Intervention strategies will include establishing or increasing the family’s linkage to other formal or informal support services in preparation for service termination.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Complete a case plan with the family developed from assessment information and family input. The case plan will be updated monthly to reflect any changes.
  2. Weekly contact with families is expected. Contact can be via phone, text, email, in-person or video.
  3. In-person contact should occur at least twice monthly.
  4. Work with the family to move toward self-management of conflicts in the home.
  5. Work with the family to identify and develop linkages to the natural supports and resources as well as formal and informal resources.
  6. Evaluate monthly the family’s response to the services provided and the development of linkages to necessary supports and resources to determine a projected closure date.
  7. Reports directly to the Child Welfare Program Manager.
  8. Work to develop rapport and establish a trusting relationship with families that will be served through the project.
  9. Attend weekly internal staffing meetings to discuss on going case management with families in the program.
  10. Participate in required staff training.
  11. Ensure data entry from home visits happen in a timely manner and assist in reporting activities as required by the Program Manager.
  12. Attend all PPI prevention team, department, and agency-wide staff meetings along with supervisory meetings.
  13. Other duties as assigned.

EDUCATION and/or EXPERIENCE:

This position requires a minimum a bachelor’s degree in social work, behavioral health discipline or related field OR 5 years field experience in a behavioral health or related setting. Mental health knowledge and/or clinical experience is preferred.

OTHER SKILLS REQUIRED:

Basic computer operational skills required. Experience in Microsoft Suite of products preferred. Strong interpersonal skills for building trust and rapport with families. Strong organizational and time management skills. Ability to maintain confidentiality of written or verbal information. Ability to work in a fast-paced environment. Must be able to lift 50 pounds.

SALARY: $40,000 – $45,000 annually.
