Job Purpose:

The Registered Nurse is responsible for working with children and families in the Child Welfare system to make necessary changes to keep children safe. The position will work effectively with a wide range of stakeholders, including families, community partners, and other healthcare professionals to provide active treatment, including a holistic approach to health care, as well as direct/coach staff regarding the diverse safety and health care needs of children in care. The RN is committed to working collaboratively and is passionate about making a positive difference in the lives of children.

QUALIFICATIONS:

  • Current Licensure as a Bachelor’s prepared (BS/BSN) Registered Nurse (RN) granted by the State of Wisconsin required.
  • Minimum of 3 years professional nursing experience in pediatrics working with infants and children with special healthcare needs required. Three to five years recent professional experience preferred.
  • Proficient clinical assessment skills with sound critical thinking ability
  • Medicaid Certified
  • Competent in the utilization of Microsoft computer programs and electronic database entry.
  • Previous home health experience preferred
  • Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
  • Must be honest, dependable and able to meet deadlines; Self-motivated and able to work independently.
  • Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
  • A valid Wisconsin Driver’s License or occupational driver’s license, reliable transportation and insurance is required.
  • Moderate exposure to noise.
  • Ability to handle multiple tasks simultaneously.
  • Ability to work under demanding conditions.
  • Good memory and recall are necessary for the accurate and timely transfer of data/information.
  • Possible exposure to blood, body fluids.
  • Exposure to outside weather conditions.
  • Regular travel in personal vehicles is required with the ability to move intermittently throughout the workday.
  • Willingness to work beyond normal working hours: evenings, nights and weekends.
  • Involved with students, personnel, family members, visitors, volunteers, interns, government agencies, etc., under all conditions and circumstances.
  • Subject to dysregulated clients, students, family members, etc.
  • Hybrid on-site/remote work is available with reliable broadband connection.

JOB RESPONSIBILITIES:

Leadership

  • Understand and articulate a vision that works to promote safety, well-being and permanency for all children
  • Actively work towards improving overall performance of the program by identifying service needs and participates in the creation of innovative, responsive service alternatives.
  • Provide leadership by representing Wellpoint Care Network  for all case reviews and staffing at the request of the BMCW to facilitate communication and understanding regarding a child’s specific health care needs and to assist in coordinating health care services for children.
  • Responsible for monitoring and communicating information regarding developmental and health related community services.

Medical Consultation/Services for All Children

  • Provide an initial RN home visit within 30 days of notification that a child has entered out-of-home care in children under age 3.
  • For all children under six months of age not enrolled in C4K, provide RN home visits every 30 days starting from initial RN home visit
  • For all children over six months of age not enrolled in C4K, provide RN visits quarterly at a minimum.
  • Provide RN home visit for all children discharged from an inpatient hospital for transitional health management within 30 days of discharge.
  • Provide a RN home visit within 30 days of reunification with subsequent RN visits reflective of out-of-home care RN visitation schedule for children under age 3 while under a court order of supervision.
  • As needed, submit a Request for Ongoing Nurse Home Visit form prior to additional visits for the approval by BMCW Health Unit Administration.
  • Provide consultation and/or ongoing monitoring to high-risk subpopulations and medically complex children.
  • As applicable, collaborate with the Care4Kids Health Care Coordination Team.

Program Development/Collaboration with BMCW

  • Participate in regular mandatory BMCW Health Unit meetings
  • Participate in the development of requirements regarding documentation and reporting of individual and aggregate data.
  • Participate in the development and provide oversight of continuous quality improvement activities including.
  • performance indicators/measures and timetables
  • Adhere to established BMCW Health Unit clinical standards and practice guidelines.

Assessment, Outcome Monitoring and Documentation

  • Identification, monitoring and documentation of acute and chronic medical needs.
  • Monitoring of physical health and growth
  • Immunization compliance monitoring
  • Observance for indications of child abuse/neglect and facilitation for referral for further medical evaluation as indicated.
  • Caregiver support regarding anticipatory guidance and health education
  • Monitoring compliance with well child health care (per the EPSDT periodicity schedule)
  • Transitional healthcare planning from hospital to out-of-home placement
  • Ensure each child has an identified primary medical home provider.
  • Document RN home visits, consultations, and/or staffings in eWiSACWIS in a timely manner.
  • Completion of required BMCW Health Unit administrative documentation including, but not limited to the development of periodic review of individualized health plan of care, and periodic updating and maintenance of applicable health information on assigned caseload.
  • Communicate with case management team regarding RN home visits or identified health or developmental concerns.
